Fee Schedule & Policies
Private Pay clients: First session $150.00 and other sessions $135.00/session. Couples sessions
$150.00.  Hypnotherapy Sessions: Private pay $150.00/session.

EAP Clients: I am no longer able to accept your EAP benefits from some insurance companies.
In the past, it has taken my office
3 months to get reimbursed for your services. Therefore I will bill
your insurance directly and you will have a co pay at the time of your session.

Insurance Billing:  I will bill your insurance directly if you have BCBS PPO, BCBS Indemnity, Harvard
Pilgrim, United Behavioral Health, Cigna, Aetna, Neighborhood Health Plan, and Fallon Community
Health Plan. Individual therapy, couples counseling, and family therapy are covered under your insurance.
If you as seeking help that will utilize hypnotherapy, this is not covered under your insurance.

Insurance Deductibles: If you have a deductible on your insurance you will need to pay for your session at
the time of your visit until your deductible is reached. The cost of the sessions will be at my private pay
rate of $150.00 for the first session and $135.00 each following session.  Once your deductible is
reached, your insurance will be billed directly.

Credit Card Fees: Our office does accept credit cards. There is a 4 percent additional charge for credit
card fees.  When you begin sessions with our office you will fill out a credit card pre-authorization form.
This will be used to pay for your private pay sessions, and any missed session fees will be automatically
charged to your card.

Missed Sessions Fees:   If you miss your appointment you will be charged a fee of $135.00 for the
missed session. If you are a hypnotherapy client you will be charged a fee of $150.00 for your missed
sessions. There is no exceptions.  I would like to make sessions available to all of my clients, and if you
do not call to cancel, then I am not able to have the other client come in when they really need to.  If you
missed more than two sessions with me, then we will need to reexamine your commitment to treatment,
and perhaps schedule you sessions at a later time when you feel that you will be able to totally invest in
your well-being.

Child & Adolescent Appointments: My practice sees children and adolescents if they have ADD/ADHD,
Asperger's, Hyperplexia, and Autism Spectrum Disorder. I also see the parent/parents of these  awesome
kids, and I will discuss with you the "homework" that would be helpful and I engage in parent "training"
counseling to help you focus on some new strategies.  Therefore you will be responsible for your
co-payments if any. Telephone consultation is not a covered benefit on your insurance. There is a charge
for telephone consults to parents and to other providers. This will be discussed with you as necessary.


Full Disclosure of Information: In order to treat you, I will need to be able to discuss freely with your MD or
psychiatrist and any other therapists, treatment providers, and will need detailed discharge summaries
from your prior hospitalizations. We can discuss your concerns during your telephone intake.

Abuse/neglect: I am a mandated reporter and will report to any appropriate agency if I feel that you
disclose where a child, elder or disabled person is being neglected. Under Massachusetts General Laws
I do not have to inform you if I am going to make a report.

Homebound Visits Travel Expense: Beginning January 1, 2009. I will begin to charge my homebound
clients a fee of $50.00 travel expense which is not covered by your insurance, and does not include your
copayment. I can discuss with you your concerns regarding this charge.
